Is it for a T5 and does it come with the slip yoke? Thanks

It was used with an automatic, but you can swap them between auto and 5 speed on the 5.0, so I don't know why this would be any different. I pulled the motor and tranny, and unbolted it from the rearend. Its ready to bolt back back in.
